Thank you for taking the time to complete this form. Whether you intend to use our services regularly or just in an emergency, completion of this form is vital so that we can care for your kids as well as possible. (We are unable to provide any care for your children unless this form has been completed)

We may share your information with emergency contacts, educators and government departments, such as the Department of Education, Centrelink, Department of Communities etc., as required and in accordance with the Privacy Act (1988), as part of permissions collected during enrolment processes.

Young Discoverers is committed to Quality Improvement. To view our Quality Improvement Plans, please see the Centre Director/Co-Ordinator. Also, we are committed to ensuring the safety of your children. All activities that carry risk will have a Risk Assessment prepared by our Educators. These are also available for your perusal at any time. Please just chat with the Centre Director/Co-Ordinator to view.

At Young Discoverers Helensvale, our Before and After School Care offers bus transport drop off and pickups only for children from Helensvale State School. We are able to accept children from other schools into Vacation Care Bookings. Should a vacancy exist in After School Care, parents/caregivers of children who attend a different school, will need to make arrangements to have their children dropped in to the Centre for their After School Care Booking

Permissions

Permission for staff to act in case of emergency or accident*

Although every care will be taken to ensure the safety of your child/ren at the Centre, the staff can in no way be held responsible for any accident which may occur. In the event of an accident or illness requiring emergency medical treatment getting your child/ren the medical support they require is our number one priority. Parents will be contacted as soon as possible in all emergency situations.

YD Recommend families seek independent advice on medical insurance as part of their enrolment due diligence

I authorise educators of Young Discoverers to administer first aid, including lifesaving intervention (e.g. Epipen, Ventolin, CPR etc.) and/or seek emergency medical treatment, including ambulance transport, for my child/ren should this be considered necessary. I agree to pay any costs involved.*

I give permission for my child/ren to be removed from the premises in the event of an emergency to a location deemed safe by the responsible person in charge or emergency services until parents/emergency contacts can be contacted, or the emergency situation no longer exists.*

Permission to administer medicine

Permission is given for my child/ren to be given paracetamol by Young Discoverers educators to reduce fever, at the directed dosage for their age and weight*

Please Note: Educators will always attempt to make contact with a parent before administering paracetamol and will document these attempts. If medication is given a parent signature will be required on the illness and medication report when your child is picked up.

Escorted journey permission

Does your child/ren over 7 years old require a booster seat?*

I understand that the children will travel in the Centre bus which has restraints for all seats, and booster seats as required. All children under 7 years of age require a Booster Seat.

Before School Care will depart from the centre at 8:30am each day arriving at Helensvale State School by 8:45am.
After School Care will depart Helensvale State School at approx 3:10pm and will arrive back at the centre by 3:30pm each day.
A penalty of $10 per child will be charged if your child has a booking and you do not inform us that he/she does not need to be picked up from school.
The route will be: Discovery Drive to Lindfield Rd, Lindfield Rd to Discovery Drive.
The journey is approximately five minutes each way.
The driver will hold an Open Class licence, First Aid Certificate and Driver Authority.
The risk assessment and policies and procedures for transporting children are available at reception.

I give permission for my child/ren travel between the Centre and Helensvale State School (Lindfield Drive, Helensvale) for Before and After school care as my booking specifies. There will be up to 10 children in the bus accompanied by two responsible adults, (comprising at least two educators, or one educator and one responsible adult). The anticipated ratio of child: adult will be 5:1.*

I understand that if my fees are not kept in advance my enrolment may be cancelled. Inclusion in holiday programs are subject to fees being paid in advance at all times.*
